On Page 1 of the Dec. 21 Monitor, a credit was inadvertently omitted on a recent photo of macroengineer Frank Davidson. Christopher S. Johnson took the photograph for the book Macro, published by William Morrow & Co. Also, the source was omitted for statistics in a chart accompanying an article on illegal immigrants in the Dec. 19 issue. The chart was based on results of a 1980 survey , ''Undocumented Immigrants: Their Impact on the County of San Diego,'' by Community Research Associates of San Diego.
